New patient forms
When you schedule your first appointment, we will send you a secure link to complete your new patient registration, health history and consent forms online. The forms take about ten minutes. If you would rather complete them in the office, plan to arrive ten minutes before your appointment.
Have not received your link, or prefer paper? Call 925-455-9510 and we will send the forms the way you like.

What to bring
- Your dental insurance card, if you have coverage
- A photo ID
- A list of current medications, including dosages
- Recent dental X-rays, if you have them (we can also request them from your previous dentist)
- Any questions you have about your teeth, gums or smile
A note on your health history
Please tell us about all medical conditions, allergies and medications, including blood thinners, bisphosphonates and any recent surgeries. This information helps us keep you safe and is protected under our privacy practices.
